Boston Scientific offers an Extended Service Agreement to continue the one-year warranty for your Boston Scientific SCS System Charging System and Remote Control if these devices become inoperable due to normal wear and tear or require additional service. Boston Scientific SCS System patients will receive a letter for participation in the Extended Service Agreement 60 days prior to the expiration of the manufacturer's warranty. The Extended Service Agreement is optional, and the terms of the Agreement will be based on your anticipated needs and past replacement experiences. If you are interested in the Extended Service Agreement, please contact Patient Care for additional information.
Some health plans cover prosthetic replacement for the Remote Control (billing code L8681) and Charging System (billing code L8689), but may have limits on how frequently they allow for ordering replacements. Check your health plan if you have questions regarding coverage and replacement of these items. Based on your discussions with your health plan provider, you may need to assess how the out-of-pocket costs for replacement of your SCS external components compare with purchasing the Extended Service Agreement.

This insurance provides limited benefits, if you meet the policy conditions, for expenses relating to the specific services listed in the policy. It does not pay your Medicare deductibles or coinsurance and is not a substitute for Medicare Supplemental Insurance.
This insurance duplicates Medicare benefits when any of the services covered by the policy are also covered by Medicare. Medicare pays extensive benefits for medically necessary services regardless of reason you need them. These include hospitalization, physician services, and other approved items and services.
